Responsible for the evaluation, treatment planning and patienteducation using rehabilitation procedures and modalities, therapeutic exerciseand home program instructions for the prevention, correction or reduction ofdysfunction. This includes goal-setting and treatment modification throughoutthe course of treatment as well as appropriate discharge planning.

Practices compliance with the State of California Physical Therapy Practice Act, the American Physical Therapy Association Code of Ethics and HealthCare Partners policies and procedures.
Assesses patient’s status both visually and manually, and observes patient’s response to treatment.
Communicates verbally and is receptive in the delivery of direct patient care.
Assumes responsibility for the care and safety of each patient during treatment.
Transports and operates various physical therapy equipment which requires the use of both upper and lower extremities and independent mobility within the treatment setting.
Performs manual physical therapy procedures including therapeutic exercise, joint and soft tissue mobilization.
Transfers and assists patients with various levels of disability and patients who are physically dependent.
Performs CPR and responds to medical emergencies.
Demonstrates competency in basic clinical skills to effectively and appropriately manage a variety of patients.
Documents and completes daily evaluations and progress notes to meet department standards of quality which includes specification of treatment goals, modification of treatment plan to achieve stated goals and treatment planning.
Participates in professional growth both within the department and through external continuing education courses which includes both giving and attending staff in-services.
Supervises and utilizes supportive staff and delegates duties as appropriate.
Assists in routine maintenance of the department and office areas.
Records patient information accurately in the medical record on a daily basis.
Maintains an appropriate patient load as determined by department standards. Alters schedule to meet department and/or patient needs.
Maintains communication with referring providers (MD, DO, NP, etc.) regarding patient’s status and physical therapy needs.
Maintains communication with co-workers, managers and providers through attendance at department, staff and provider meetings.
Uses, protects, and discloses HCP patients’ protected health information (PHI) only in accordance with Health Insurance Portability and Accountability Act (HIPAA) standards.
Performs additional duties as assigned.
Bachelor’s degree from a four-year college and/or a professional certification requiring formal education beyond a two-year college.
Degree in Physical Therapy from an accredited university.
Current California Physical Therapist licensure.
Master’s degree or greater in Physical Therapy preferred.
Over 3 years and up to and including 5 years of experience or completion of clinical residency program.
Over 5 years of experience with clinical specialization.
2 years of clinical experience in a managed care or comparable setting.
Previous experience with teaching students or public speaking.
Diverse background of continuing education and clinical experiences.
Basic Life Support for healthcare providers.
Strong clinical and decision-making skills.
Ability to work and effectively manger patient care in a fast-paced setting.
Effective verbal, written communication and interpersonal skills.
Ability to work as a team player.
Ability to be adaptable and flexible in daily work schedule as department needs require.
Advanced clinical skills or specialization preferred.
