Overview:

The Regional Clinical Pharmacist isresponsible for maintaining the retail pharmacy customer service standards.

Essential Functions:

  • Conduct one-on-one comprehensivemedication reviews (CMR).

  • Review hospital census reports dailyfor patients in assigned region and complete comprehensive review to includeadherence of all maintenance meds.

  • Complete medication reconciliationupon hospital discharge to ensure education, procurement and compliance are inplace.

  • Follow up with patients to ensureadherence to recommendations made during CMR’s and/or discharge medicationreconciliation

  • Assist individual clinics with STARsmeasures and other clinical goals

  • Provide disease state groupeducation as required within assigned region.

  • Work with clinics to resolve anymedication issues including pricing, procurement, best practices, etc.

  • Attend huddles with clinic staff

  • Attend regular meetings withRegional Medical Director and Center Support Managers

  • Attend POD (regional) meetings asscheduled

  • Review cost-saving opportunities andfollow up with clinicians as necessary.

  • Discuss and follow up withphysicians on any prescriber-related recommendations

  • Attend other meetings and trainingsessions as required

  • Serve as preceptor to pharmacyexterns as required

  • Occasional retail float pharmacistas needed

  • Other projects and activities asassigned

Qualifications

* Education: *

  • Active Florida Pharmacist License ingood standing.

  • Must maintain all CE requirements asper state laws

* Experience: *

  • Working knowledge of current drugtherapies and protocols - Prior DUR experience desirable

Knowledge, Skills, and Abilities:

  • Strong planning skills,self-motivated with ability to organize own work

  • Ability to read, write, speak andunderstand the English language fluently

  • Ability to work and communicateeffectively with co-workers, clinic staff and physicians

  • Work effectively and professionallyunder pressure

  • Computer skills with a variety ofsoftware programs (especially Excel) and knowledge of Internet capabilities toassist in researching changes in therapies and protocols.
