We are seeking a CareCoordinator- Care Management to join our talented team!

Overview:

The Care Coordinator assists and supports the CareManagement Team with daily operational tasks, data input and collection,clerical and administrative duties.

Essential Functions:

  • Completesadministrative functions associated with Care Management tasks

  • Assistspatients with external resources at the direction of the Care Management nurse

  • Utilizesvarious data sources and enters data in JSA programs and spreadsheets

  • Maintainscall logs, excel spreadsheets, and data findings electronically

  • Compiles,organizes, and mails out materials to internal and external customers

  • Performsgeneral clerical duties such as typing correspondence, creating or updatingspreadsheets, photocopying, faxing, and compiling data and other materials orother duties as required

  • Obtainsnecessary patient information from clinical centers in order to assist withdischarge process

  • Obtainsmedical records and faxes to Scanning department for entry into EHR

  • Entersreferral requests

  • Assistswith scheduling post discharge follow up appointments with PCP when requested

  • Otherduties as assigned

Qualifications

* Education: *

  • Highschool graduate, GED or equivalent

  • Graduationfrom an accredited Medical Assistant Program.

* Experience: *

  • MedicalAssistant experience, RMA/CMA Preferred

  • Experiencein a health care related setting

Knowledge, Skills, and Abilities:

  • Abilityto communicate effectively with patients and all levels of personnel in a professional,courteous and effective manner using excellent customer service skills

  • Possessexcellent communication and organizational skills with the ability to multi-task,set priorities, and meet deadlines

  • Resultsand goal-oriented with a philosophy for quality improvement

  • Mustbe computer literate and possess the ability to master different medical recordsoftware programs

  • Demonstrateexceptional telephone skills

  • Demonstratedproficiency in Microsoft Office, Email and general computer functions

  • Proficientin Excel spreadsheets

  • Strongcommunications skills

  • Possesshigh degree of accuracy, efficiency, and dependability

  • Abilityto perform multiple tasks and possess strong organization skills

  • Abilityto follow written and oral instructions and to work with general guidance

  • Knowledgeof HIPAA regulations

  • Musthave consistent, punctual, and reliable attendance

  • Abilityto travel may be required

  • Abilityto read, write, speak and understand the English language fluently
