DaVita Social Worker -Case Management Team in Philadelphia, Pennsylvania
This Social Worker will be a part of our new & growing VillageHealth Case Management team ! You will be a pioneer, as the first social worker for this program! The social worker will support our team in the greater Philadephia area, West Chester county, and parts of New Jersey.
The Social Worker provides guidance, assessments, support, and information to patients and their families to maximize the psychosocial functioning and adjustment of the patient in chronic kidney disease, including but not limited to, recommendations about financial programs, medical assistance programs, vocational rehabilitation programs, transportation resources, mental health and chemical dependency resources, and home health agencies.
MVP interventions & Weekly IDT call interventions.
Counsels new and current patients and families on treatment options and modalities; provides support, information, and assistance in treatment planning and adjusting to dialysis
Independently analyzes and provides information and facilitates referrals to community, private, state, and federal agencies based on psychosocial needs (e.g. financial programs, medication assistance programs, vocational rehabilitation programs, transportation resources, mental health, and home health agencies)
Acts as team subject matter expert for Motivational Interviewing.
Evaluates and monitors ongoing psychosocial issues and patient goals; communicates with interdisciplinary team regarding patient psychosocial issues affecting treatment and well being

Master’s degree in Social Work (MSW)
Licensed or registered in the state of practice as required by state regulations
Demonstrated knowledge of government and private insurance programs
Intermediate proficiency in MS Work, Excel, PowerPoint, and Outlook
Experience working with kidney care patients
