The Clinical Care Liaison is responsible for the development and expansion of our post-acute management program establishing relationships and facilitating processes with local hospitals and skilled nursing facilities.This position educates hospital administration, physicians, and discharge planners on high quality facilities available for post-acute care. The Clinical Care Liaison works collaboratively with the Post-Acute team to ensure seamless admission and discharge processes are met.This position reports to the Director of Operations.

Essential functions of the Clinical Care Liaison role will include:

·Educatehospital and skilled nursing facility staff on post-acute care programs.Will be responsible for independently developing relationships and managing communications with key clients

·Evaluate gaps in post-acute discharge processes with hospital partners and implement process plans to address any areas of concern

·Identify discharge and admission issues and collaborate with hospital clients and teammates to generate creative solutions

·Develop and implement new ways for DaVita to differentiate our post-acute care programs in the market

·Uses, protects, and discloses patients’ protected health information (PHI) only in accordance with Health Insurance Portability and Accountability Act (HIPAA) standards.

·Performs additional duties as assigned.

·Bachelor’s degree REQUIRED

·At least two years or more in a hospital or other healthcare setting REQUIRED

·Minimum of two years customer service or marketing experience in healthcare REQUIRED

·Experience with geriatric or end of life care preferred

·Proficiency in Microsoft office and ability to type 25wpm

·Deep knowledge of community resources and commitment to innovation

·Ability to interface effectively with culturally diverse patients and communities, providers, management, and employees of the organizations.

·Ability to exercise objectivity and good judgment relating to difficult and emotionally-charged situations.
