House Calls Behavioral Health Consultant

The house calls behavioral health consultant will join our growing team in the Philadelphia area focused on improving the healthcare of patients with the most intense and complex medical needs, including but not limited to end stage renal disease.This position will provide a combination of in-home medical care and telephonic medical care to members with documented or suspected behavioral health conditions.Behavioral health can dramatically impact medical health and this role will be a vital part of our house calls care team.This position will collaborate with physicians, clinical care team, patients and their families, and other healthcare professionals to address the patient’s mental health needs.
Essential functions of the House Calls Behavioral Health Consultant role will include:
Perform initial comprehensive mental health assessment for members and conduct additional patient assessments as necessary.
Collaborate with clinical team to develop a comprehensive care plan including the patients mental health needs.
Coordinate directly with patient and their care providers to discuss changes in condition, potential medication changes, and modifications to care plan.
Implement interventions to support the patient to regain or maintain mental well-being and monitor the effectiveness of prescribed interventions.
Serve as expert for mental health resources to the team and maintain list of available resources for patients.
Participate in the development of clinical practice guidelines regarding patient behavioral health conditions.
Collaborate with other clinical and administrative staff in a positive, constructive manner.
Use, protect, and disclose patients’ protected health information (PHI) only in accordance with Health Insurance Portability and Accountability Act (HIPAA) standards.
Perform other duties and responsibilities as assigned or requested.

Completion of accredited program in Psychiatric/Mental health such as a nurse practitioner (PMHNP) or M.S in counseling degree
Minimum of two years clinical experience, home or hospice based care preferred
Extensive knowledge of mental health resources in the Philadelphia area
Experience performing patient evaluations and consults telephonically preferred
Excellent organizational and planning skills
Experience working telephonically or with geriatric populations preferred
