This

Divisional position manages the Development of Admissions, as well as oversees

and supports our Kidney Smart program, including, but not limited to, market

outreach to hospital partners, teammate process education, and quality control

management.

Responsibilities

include, but are not limited to:

The

following duties and responsibilities generally reflect the expectations of

this position but are not intended to be all inclusive.

Admissions

·

Assists

with necessary communication between hospital and clinic teammates to work

through patient placements with extenuating circumstances

·

Directs

the formation of Admission Teams and support teammates (FA, AA, SW)

·

Partners

with Hospital Discharge Planners in facilitating smooth discharge/admission

processes

Marketing and

Outreach

·

Partner

with field teammates to strategize and implement cost effective marketing tools

where appropriate

·

Partner

with Business Development Team and Regional Operations Directors on appropriate

doctor and hospital outreach efforts

Communications and

Customer Service Skills

·

Ensure

proper training of facility staff on the importance of accurate data collection

and how to complete the following forms:

Patient Admission and Discharge, Medicare Screening, Insurance Verification,

Application of Medicare Entitlement Due to ESRD, Financial Evaluation

·

Manage

the process by working with patients, teammates, and hospital or medical office

staff to gather patient information and respond to any concerns with patient

insurance appropriately

Other Duties

·

Fulfill

other duties as assigned or needed

·

May

be required to work overtime with limited or no notice

·

Understand

and follow DaVita patient and teammate policy and procedures

·

Comply

with DaVita policies and procedures in accordance with State and Federal

Regulations for patient care and services. (Medicare, HIPAA, OSHA or other

governing agencies)

·

Comply

with State or Federal Regulations of the Injury Prevention and Safety Training

MINIMUM

QUALIFICATIONS

Education,

licenses, certifications, and experience required to fulfill the essential

duties, include computer skills as required.

Bachelors

Degree preferred

·

Ability

to travel to various locations to meet with partners (internal and external) on

an as-needed basis. Includes valid driver’s license in state of residence and

insurable driving record

·

Intermediate

computer skills with a variety of software packages including but not limited

to Microsoft Word, Excel, PowerPoint, billing systems
