Overview:

Responsiblefor providing integrated support under the supervision of a pharmacist toHealthCare Partners clinicians and patients by managing refill requests andprior authorizations, conducting therapeutic drug conversions and supportingpharmacy wellness programs

Essential Functions:

  • Consistently exhibits behavior and communicationskills that demonstrate HealthCare Partners’ (HCP) commitment to superiorcustomer service, including quality, care and concern with each and everyinternal and external customer.

  • Works as a member of the Pharmacy Support Center byprocessing medication refill requests per protocol, facilitating non-protocolrefill requests, obtaining prior authorizations and resolving other medicationrefill related issues or problems.

  • Documents pertinent patient information in theelectronic medical record.

  • Participates in therapeutic drug conversionprograms by conducting therapeutic drug conversions per protocol, documentingdrug conversions in the electronic medical record, notifying providers of drugconversions and providing patient education regarding these changes.

  • Assists with data management and recording resultsfor month-end reports.

  • Assists in Pharmacy Services wellness programs (i.e.anticoagulation clinic and disease management clinics) by:

  • Scheduling patient initial and follow-upappointments

  • Retrieving pertinent patient information for newpatient referrals or recent hospital/SNF discharge

  • Rooming patients

  • Checking vital signs and performing basic lab test(i.e. PT/INR finger stick)

  • Contacting patients regarding lab results andinstructions

  • Performing patient “Missed Appointment / No Show” procedures

  • Answering incoming phone calls and directingpatient messages

  • Maintains, stocks and cleans exam rooms.

  • Updates job knowledge by participating ineducational opportunities and reading professional publications.

  • Participates in process improvement initiatives, sitestaff meetings and departmental meetings.

  • Uses, protects, and discloses HCP patients’protected health information (PHI) only in accordance with Health InsurancePortability and Accountability Act (HIPAA) standards.

  • Adheres to Universal Precautions and safetypolicies and procedures at all times

  • Performsadditional duties as assigned

Qualifications

EDUCATION:

  • 1 or 2 years of post-high school education or adegree from a two-year college.

  • Current California State Board of PharmacyTechnician license.

  • PTCB certification

EXPERIENCE:

Minimum:

  • Up to and including 1 year of experience.

Preferred:

  • 1 year experience in hospital, ambulatory care orother healthcare setting.

  • Prior experience in managed care setting.

KNOWLEDGE, SKILLS, ABILITIES:

  • Computer literate.

  • Ability to log on to computer and using keyboard,mouse and email applications.

  • Knowledge of Windows and MS applications (Word,Excel).

  • Ability to type 40 wpm.

  • Basic knowledge of medical and pharmacy terminology.

  • Ability to demonstrate basic knowledge inmedication therapeutic categories.

  • Ability to identify generic name for brand namedrugs and vice versa.

  • Ability to demonstrate basic knowledge inunderstanding indication for medication.

  • Excellent organizational, interpersonal anddecision-making skills.

  • Excellent verbal and written communication skills

  • Ability to work in a relatively unsupervisedenvironment with a high degree of accuracy.
