Overview:

Under minimal supervision, supports the development and delivery of cost effective coordinated care and utilization management programs that assure cost-effective use of resources and high quality of care in the inpatient, ambulatory and SNF settings. To advise and consult hospital care managers, clinic care managers and physicians re: appropriate services to meet patient's care needs. Collaborate with the appropriate care coordination colleagues to ensure appropriate referrals and authorizations are aligned for patients throughout their transitions for key services needs. Utilizes nursing process, standard work, and critical decision making skills to achieve district and individual patient goals. Works closely with primary care-based RN care managers and consultant care staff (eg: anticoagulation, palliative care, specialty departments) to provide efficient and well designed care transitions. Assist in the development of new collaborative approaches which will reduce unnecessary service and cost utilization.

This is an exempt level position (1.0 FTE)

Qualifications

Knowledge:

Knowledge of professional nursing theory and practice to assess, plan, implement and evaluate patient care. Knowledge of ICD and CPT coding preferred; knowledge of medical terminology and managed care required. Knowledge of organizational policies, regulations and procedures to administer patient care. Knowledge of medical equipment and instruments, common safety hazards, and precautions to establish a safe work environment.

Skills:

Skill in identifying problems, researching and recommending solutions. Skill in preparing and maintaining records and establishing and maintaining effective working relationships with patients, medical staff and the public. Good professional communications and interpersonal skills. Strong organizational skills to set priorities and ability to work with high degree of accuracy and attention to details while responding to tight deadlines and multiple priorities and demands.

Abilities:

Ability to maintain quality control standards. Ability to react calmly and effectively in emergency situations. Ability to interpret, adapt and apply guidelines and procedures. Able to manage, advise, direct and evaluate clinical care coordination staff in care coordination unit/department, as needed.

Education:

RN, Bachelor of Science in Nursing, preferred

Experience:

Minimum of two years of professional nursing experience required. Ambulatory and inpatient experience desirable. Comfortable in working with a variety of clinical guidelines/criteria for appropriateness of care along the care continuum.

Certificate/License:

A Washington State Registered Nurse license, and BLS certification through American Heart Association. Case management certification desirable. A Washington State driver’s license and automobile are required.
