We are currently seeking a Director to join our high-energy, creative and highlymotivated Transformation Team - Catalyst. This team is dedicated to highvisibility and high-value projects inside and outside HealthCare Partners.

Most Catalyst projectsare focused on helping a partner succeed in value-based care. Some of thecurrent projects include:

  • Building value-basedcare capabilities for partners, including but not limited to:

  • High-risk programs

  • Transition of care programs

  • Readmission reductionprograms

  • House calls and otherpost-hospitalization solutions

  • Clinically IntegratedNetwork

  • Physician incentiveprograms

  • Best practicecomprehensive health assessment programs

  • Reporting and analytics

  • Leading marketclinical/operational performance improvement efforts

ESSENTIAL FUNCTIONS:

  • Setappropriate target goals for department, implement action plans to achievethem, and monitor results

  • Ensure thatmetrics being tracked are relevant to program success

  • Review,authorize, and monitor expenditure reports in accordance with company policyand procedure

  • Perform allrelated teammate management activities including but not limited torecruitment, retention, hiring, promotions, performance evaluations, salaryincreases, and disciplinary actions with appropriate approvals

  • Develop andmaintain positive external relationships with potential business partners

  • Up to 50% Travel

MINIMUM QUALIFICATIONS:

  • Bachelor’sdegree in Business or related field required

  • HealthcareConsulting background a plus!

  • Master’s inBusiness Administration or Masters in Healthcare Administration preferred

  • Minimum of 2years’ business operations experience required

  • Minimum of 2years’ supervisory experience required

  • Experiencein process and change management preferred

  • Intermediatecomputer skills and proficiency in MS Word, Excel, Outlook, and PowerPointrequired
