Program Manager, Comprehensive Health Assessment Programs (CHAPs) (CSHP)


Summary: Support the planning and coordination ofnational initiatives and operational programs related to Medicare RiskAdjustment. Lead national initiatives and support teams engaged in riskadjustment analytics, patient outreach, documentation integrity, clinicianeducation and tools, and enterprise audits.

EssentialDuties and Responsibilities include the following, other duties may beassigned:

  • Project management of multiple initiatives and activities, both at the national and market levels.

  • Define, implement, lead and measure processes from start to finish.

  • Ensure project deliverables, metrics measurement, and success criteria achieved.

  • Support national and market patient engagement, outreach, provider education programs to facilitate comprehensive health assessment.

  • Coordinate implementation of programs designed to ensure all diagnosed codes and conditions are properly supported by appropriate documentation in patient charts.

  • Oversee clinician chart audit activities and coordinate operational aspects of clinical chart reviews.

  • Develop executive level materials and effectively present to executive audiences.

  • Develop strategic messaging and communication plans in partnership with Directors and others for each initiative.

  • Participate in the development and creation of training materials (e.g., job aids, presentations, etc.).

  • Deliver training to various teammates and other stakeholders with fluency and credibility, facilitating a necessary understanding of key objectives.

  • Assess, respond and react to CMS and/or internal mandates to ensure compliance and programmatic success.

  • Synthesize complex programs and clearly communicate them to a wide range of stakeholders.

  • Analyze patient and clinician data to monitor performance, validate outcomes, and identify improvement opportunities.

  • Review CMS documents related to risk adjustment, synthesize findings, and formulate recommendations.

  • Uncover insights from analyses that can be leveraged strategically, clinically or operationally.

  • Attend internal team meetings and call to ensure alignment around project objectives, deliverables, and timelines.

  • Maintain knowledge of company policies and procedures, OSHA and other related regulations and guidelines, and ensure department compliance.

  • Use, protect, and disclose CSHP patients’ protected health information (PHI) only in accordance with Health Insurance Portability and Accountability Act (HIPAA) standards.

  • Attend and complete all required training and development to include annual compliance training.

  • Perform additional duties as needed.

Bachelor'sdegree from a four-year college preferred. At least 3-5 years of projectmanagement and process improvement experience in a healthcare setting.Experience working with health plans or large physician group preferred.Knowledge of Medicare Advantage and population health activities preferred.

Certificates,Licenses, Registrations:

CPHQCertification preferred.

  • Demonstrated success working in dynamic, fast-paced environment.

  • Demonstrated success leading projects from ideation to closure.

  • Experience conducting root cause analysis and identifying optimum solutions.

  • Excellent verbal and written communication skills.
