DaVita Paladina Health- Regional Clinical Care Coordinator in Cleveland, Ohio


Health’s Clinical Care Coordinators play an integral part in driving our

clinical outcomes and works directly with the other Care Coordinators, the Care

Manager and the Clinical Operations team with direction from the Chief Medical

Officer in support of our care management goals. The Clinical Care Coordinator supports the

clinics by creating and managing registries of at-risk patient populations

using our EMR and claims data, supporting our clinic care teams in driving

targeted intervention and care plans, and executing cross-cutting clinical

initiatives across their network of clinics.

Paladina Health builds and manages

employer-sponsored primary care health physician offices around the

country. Launched from scratch in 2011

The following duties and responsibilities generally reflect the

Clinical Care Coordinator will have several core responsibilities:



physicians and the care team develop patient care plans and goals based on

clinical quality care and population health management registries. Prioritize patient outreach and follow up as




collaboratively with physicians and the care team to ensure patient adherence

to medical plans of care, including all appropriate preventive and disease-specific

screenings, interventions, and treatment goals including self-management goals



and controls the task, phases, and deliverables of population health and

clinical initiatives based on the identified project goals and objectives



in training programs to share best practices



patient level data to understand trends in individual clinic patient populations

and identify areas for improvement



management of new initiative execution across designated clinics



with high risk patient populations to drive outreach, assist in care planning,

and provide motivation and education where necessary.



patients in navigating the healthcare system while being a steward of the

  • apatient-first cooperative attitude and a willingness to go the extra milefor patients

a clinical degree with clinical experience and

background. RN required

  • 3or more years of direct patient care experience and/or experience in amedical office setting
  • caremanagement experience in medical group, health system, or health plan
  • experienceworking in population health management strongly preferred
  • ahigh level of comfort with IT systems (EMR, practice management systems)
  • aself-started or self-directed mentality
  • analyticalability to summarize data, highlight exceptions, view trends
  • abilityto manage care teams through a project and its deliverables.
