OVERVIEW OF POSITION
Responsiblefor developing and implementing the outpatient diabetes program for HealthCarePartners (HCP).
Consistently exhibits behavior and communicationskills that demonstrate HealthCare Partners’ (HCP) commitment to superiorcustomer service, including quality, care and concern with each and everyinternal and external customer.
Participates as a member of interdisciplinaryhealthcare team to facilitate the continuum of care and education of patientswith diabetes.
Monitors and evaluates, as needed, resourceutilization, patient outcomes, patient’s skills in self-care, and patient’sability and readiness to learn.
Counsels and educates individual patientappropriately on diabetes care in conjunction with patient’s medical provider(medication adjustment, exercise, diet, sick days, etc.)
Evaluates blood glucose records to determineefficacy of current medication therapy, solve hypo/hyper-glycemia problems, andmake needed adjustments.
Ensures correct and timely documentation inpatient’s medical record following individual consultation.
Develops and conducts group presentations andrelated educational programs as needed.
Develops and maintains patient education materialsrelating to diabetes.
Serves as a resource for clinic staff, home healthand other departments.
Uses, protects, and discloses HCP patients’protected health information (PHI) only in accordance with Health InsurancePortability and Accountability Act (HIPAA) standards.
Performs additional duties as assigned.
Bachelor’s degree from a four-year college and/or aprofessional certification requiring formal education beyond a two-yearcollege.
Graduation from an accredited school of Nursing.
Current California RN license.
Certification as a Certified Diabetes Educator.
Bachelor’s degree in Nursing preferred.
- Over 3 years and up to and including 5 years of experience.
- 2 to 3 years experience in diabetes care.
KNOWLEDGE, SKILLS, ABILITIES:
Microsoft Office applications skills (Word, PowerPoint).
Ability to type 25 wpm.
Effective communication and teaching skills.
